If she didn't sign a release it won't make it into an episode. They shoot more than than can legally show.
If RW were a news show, they could use it. Since it's entertainment programming, however, they need the consent of anyone materially participating in a scene. I'd say she unquestionably qualifies as "materially."

Sure, but what would be the point? I assume her words are critical to what is happening. She owns her words in addition to her likeness. Without her consent, production would need to remove both her image and her words... What's left?
In TV production, we blur out the faces of people in the background when we don't have a release for them or they have otherwise entered a private facility where access points have been properly posted with notice of filming.
Maybe it'll be one of those instances where we only get the summary of what happened through an interview/confessional with Frank and any other cast member involved - then we watch Frank vent about the situation to the cast.

Possibly, but that approach seems rather unsatisfying.
There is usually a crew member responsible (and good at) obtaining written approvals from people who encounter the cast in this way. It's how they got Reva to sign during RW Hollywood (even though she was drunk and incapable of consent, according to her).

She is not from Boston, she doesn't have a kid, so I guess she doesn't have any kind of serious illness, since its something that they deal at the time they were appearing on their season? Maybe an eating disorder?

Hahaha for the record that's exactly why I figured that wasn't it. I almost put "because I don't know how molds would know that" but then I thought maybe it wouldn't be the craziest thing we've learned about somebody before the season airs.
