I think we maybe underestimating Derrick's willingness to screw over Johnny. The Challenges he did with him were Team Challenges. Derrick did screw over Theo on FM to better his chances of a win. I can see him doing that to Johnny. Especially if Johnny has a better partner than him.

I think Nicole would accept only if she knew Nate wouldn't be on. She already said she would decline for Exes III, basically meaning she didn't want to be around Nate. the only reason I see her accepting is if production tells her Nate won't be there. So either it was a coincidence that she was in NY (or was she in LA?) at the time, or she knows Nate won't be on the season.

It sucks Cooke debuted on Rivals 2. Production was really awful to the cast that season with living conditions (no AC in that horrible weather) and it might have turned her off. I would have preferred her debuting on Seasons 2, then returning for Free Agents.
