4. For the most part, yes. But if they have two very castable people who happen to be from the same state, then by all means cast them. And Skeletons did a good job of this - they were all from different states.
5. I really like what they've done in recent seasons - asking the victim in the fight whether he or she thinks the instigator should be sent home. I feel that there are a lot of factors that go into this, but if the fight was relatively minor and nobody feels unsafe, they don't need to send anyone home

No however towards number 7. Recasting cities is lame. If they do subtitles then maybe because most people that watched Ex-Plosion and Skeletons weren't aware where they were save for some of the episodes like the Bean and the premiere.
