I'm fairly certain Kurt's coming out process really only lasted about four episodes (the first four of the series). He came out pretty quickly and didn't really have a struggle with who he was and have to try to accept it. He knew who he was, even if he didn't want to admit it. The turmoil associated with Kurt's story lines is that he is bullied for being gay and it's not about him try to figure out who he is and come out of the closet.
I think Karofsky is more in denial, and showing a coming out process involving him accepting who he truly is would be more interesting/real than Kurt's original story line.

I am not sure about the death thing other than it most certainly won't be any of the characters I want killed off (Kurt and Quinn).
The only thing I know re: Sam was some blog had some onset photos with his family and apparently he is supposed to be like the oldest of a huge family as there were multiple younger kids in the photos. Seems like a mom was mentioned but not a dad.

I'd be fine with Quinn being that character who dies.... although I doubt that'll be the case. I see them killing off Santana before Quinn, (even tho I like Santana way more). It could be Tina or something. Idk.

My two ideas I've got going were either
1) his parents are a same-sex couple - and I figured it would be two women since Rachel already has two dads
or
2) one of Sam's brothers has down syndrome. I've read that there was a male character cast who has down-syndrome but there wasn't any further information on him. Just popped into my head that he could possibly be Sam's brother.
as far as character deaths go... I'm unfortunately going to guess that it's Terri Schuester... just because they don't really use her much. And by killing her, Will can head into a depression for season three and prolong his and Emma's relationship even longer. D:

The first thing that popped into my head when I read "Seems like a mom was mentioned but not a dad" was something to the effect of his dad left his mother and he is now the "father" of the family, and has to be responsible for taking care of all his siblings.

Mike O'Malley's signed on for season three, from what I've read.
I would be REALLY sad if they killed off Carole. I really, really love that character. I can't remember which episode it was, maybe "Theatricality," but it's the one when Burt and Carole first move in together and Kurt and Finn share a room. There's a scene prior to them moving in when Carole and Finn argue and Carole throws the urn onto the recliner and went on about how Finn wants to pretend that nothing's changed and all that fun stuff. Romy Rosemont knocked that scene out of the park. It was like, the best scene out of those final nine episodes from season one. That's the one thing that'll always stick with me from the second half of season one. Now if I could only remember the name of the episode... :/

She did do an amazing job with that scene. I believe that's from the Home episode. Theatricality was the Gaga ep.
Someone posted in a comment on a spoiler thread that the actress who plays Coach B has signed onto do some new Chelsea Handler show. I am not sure if that would be a major character, and she maybe could still do both shows, but that seemed like maybe a possible victim. Kurt and Blaine are not going to break up because Darran Criss is onboard for Season 3, so I think maybe the break up has to be Finn's.

That's my thinking too. They have never really known what to do with Sam once he wasn't Kurt's BF so this is probably their way of spinning that character a new storyline in order to keep Chord Overstreet on the show. IMO, as inconsequential as Sam is, Chord has the best potential to break out of the series earlier than the rest of the current cast. That he has been romancing some known ladies means the kid is well-connected and could likely do movies pretty easily if he decided he didn't like sticking around Glee.
